Instructions

We suggest lining each box being prepared for shipment with a plastic bag and sufficient packing material to prevent breakage. Packing material can include newspaper, packing peanuts (contained in a bag), bubble wrap, etc.

Make certain that ALL bottles and jars are CAPPED TIGHTLY to prevent spillage and leaking!

Package your outdated Legend drugs into the boxes which have been prepared for shipping. Remember, it is necessary to inventory the outdated Legends.

When returning Control III-V drugs, you may ship them in the same box as your Legend drugs as long as they are contained in the provided tamper evident bag. Controls cannot be intermingled with OTC/Legend.

Place any necessary forms, and the Control Drug Report into the packing list envelope and seal.

Attach the packing list envelope to the outside of one of the boxes. If you are returning Controlled substances, attach the packing list envelope to the box containing the Controlled substances.

We ask that you please number the quantity of boxes being returned on the return address label, i.e., 1 of 3, 2 of 3, 3 of 3.

CII Returns

1. Fill out 222 Request Form
2. Fax to: 630-277-8340
3. Form 222 & labels arrive by mail in a week or so
4. Follow instructions provided with Form 222

3 Box and ship your order

1. Pack safely in a box (see shipping details above)
2. Put any/all forms in packing list envelope
3. Fill in FedEx label
4. Put ALL the labels (3) on outside of box(s)
5. Call 888-777-6040 for FREE "PRP" Pickup
6. Keep a record of your of your tracking numbers.
7. Schedule II items MUST be shipped separately from all other items.

CIII-V Returns

2. Put CIII-Vs in supplied tamper-evident bag

3 Box and ship your order

Legend/OTC/HazMat Returns

1 Pull your outdates 3 months ahead and prepare an itemized list of what is being returned using your customer portal legend drug entry program.

8. All legends and scheduled drugs must be itemized before shipping.
