Returning Scheduled Drugs

New Customer Information

In order for us to better serve you; we need to know a little bit about your facility. This form provides all of the necessary information about your facility in order to most effectively process your returns and expedite your credits.

Wholesaler Change

It is important for us to have current wholesaler account information. This guarantees prompt and accurate return credits. Please let us know if you add a secondary wholesaler or make any changes to your main wholesaler account.

Schedule II Return Request

In order to return your schedule II drugs you must order a DEA Form 222. Simply enter on-line and submit a Schedule II Return Request or print out the form and fax to 630-277-8340. Within 24 hours we will issue a DEA Form 222 with instructions and a prepaid shipping label. Refer to the Send a Return link for further instruction.

Schedule III - V Control Report

Our On-line Control Drug Report will help you organize any schedule III- V drugs which need to be destroyed or returned for credit. Simply enter your schedule III-V’s on-line and then print your Control Drug Report for use with your schedule III-V return. You may also print the PDF version of the form and manually fill it out for use with your schedule III-V return. Refer to the Send a Return link for further instruction.

National DEA Offices

When returning schedule II drugs you will need to mail the green copy of the DEA Form 222 to your local DEA office. Refer to the Send a Return link for further instruction. This is a list of all the DEA field office locations, and the states that they serve.

Sample Report Request

If you submit a request for our sample reports, we will email or fax you a copy of our 16 page sample reports within 24 hours. In addition, we will have one of our return technicians call you to review the reports to explain their content.

Form 41
